full spectrum NDIS provider in home & community

SIL (Supported Independent Living)

In-Home Supports (Assistance with Daily Life)

Supported Independent Living or ‘SIL’, is what NDIS calls the supports a person needs for their jobs around the home.

It’s for the things we do every day, including:

· personal care, like brushing our teeth or showering

· cooking and meal assistance

· cleaning

· shopping for groceries and personal items

SIL support focuses on developing the Participants skills to help them live as independently as possible.

SIL funding pays for the support workers who provide this assistance. It’s most often used in a shared home, for people who need support available 24/7.

It doesn’t cover rent, groceries, transport, or utility costs like gas and electricity.

A person might use SIL funding to live in:

· a place they rent privately

· the home that they own

· a home they share with others

· Specialist Disability Accommodation (SDA), if they have it in their NDIS funding package

We are an NDIS Registered SIL Provider, this means we take our role in your home seriously.

In-home support is just that, support in your home. Our support team tailors their care to your needs. If you need assistance doing the dishes or hanging out the washing, we’re here to help. If you aren’t able to clean the house or need help to get ready in the morning, we’re here to help you.

Community Access (Assistance with Community and Social Participation)

Sometimes, it’s just nice to go out and grab a coffee with someone who has similar interests as you.

Sometimes, you just want some company to go for a walk, and sometimes you might want to build your confidence trying some new things.

Our support team is motivated and keen to help you get out there more.

We love learning more about our participants and what interests them most. Whether that’s attending the Portarlington Mussel Festival, Pako Festa or just going to the movies, we’re all about accessing the community.

Do you need some extra support getting to doctor’s appointment? Do you have a regular social event you find challenging to get to without someone going with you? Do you sometimes get worried you’ll be attending the event alone?

Our support team is here for you. We provide support to attend appointments, social and community events. Whatever you need, we’re here to help you.

We keep up to date with local community events and activities that might interest some of our participants, as well as networking with our community for any new and upcoming events!

Support Coordination (Level 2)

Support Coordination is all about working with you to ensure you have choice and control over your NDIS plan and the types of providers you wish to engage with. Support Coordinators are an NDIS stated support.

They help to implement your NDIS plan; they work with you, your NDIS Planner/LAC, and your Care Team to set up the supports outlined in your NDIS plan.

They help to engage you with service providers (e.g. support workers, Allied Health therapists, and holidays). They help to allocate your NDIS budget to your chosen providers to help you get the most out of your NDIS plan.

They write reports to NDIS to outline how you and your supports have been over the plan period and discuss with you what your NDIS goals are. Your Support Coordinator also helps you to engage in mainstream and community supports as well.

Main focus:

• Supporting you to understand more about your NDIS plan and the NDIS

• Helping you to implement your NDIS plan

• Source and provide a range of service providers to choose from

• Speak with service providers and take the hassle out of organising

referrals/equipment requests

• Source mainstream and community supports

• Help you to prepare for your Plan Review with NDIS

• Write reports for NDIS and ensure your Allied Health team and any other providers supply their evidence/quotes for next plan.
